Sealant on floor under the oven has worn, exposing concrete, which is no longer a smooth, easily cleanable surface. Food employees not wearing proper hair restraints, such as nets or hats. Clean, plastic containers stored wet in a manner that does not allow for air-drying (wet-nesting). In walk-in freezer, employee's frozen dessert found stored on same shelf as facilities food rather than segregated and labeled. Food employee donning single-use gloves without a prior hand-washing. An employee's open beverage container was on bain-marie cutting board, a food preparation area. Two very deep crevices on cutting board that does not allow for effective sanitizing. Repair or replace. The front hand-wash sink is being used for multipurposes as evident by a wiping cloth and vegetables in drain of the sink.
