SE Health Services gets $50,000 grant for asthma, diabetes help
SouthEast Lancaster Health Services recently was awarded a $50,000 grant to help people with asthma and diabetes.
The grant is an Innovations in Community Health grant from the CVS Caremark Charitable Trust, in partnership with the National Association of Community Health Centers.
The center will use the funds for its asthma or diabetes Patient Wellness Circles. The circles bring together patients with the same diagnosis for education, discussion and support.
SouthEast Lancaster Health was one of 21 community health centers nationwide to receive a grant from CVS Caremark Charitable Trust. The grants support the development of programs that focus on chronic illnesses, and help centers increase access to health care and produce better outcomes while reducing costs.
