The Pennsylvania Department of Agriculture, 866-366-3723, uses a "risk-based" inspection reporting process for restaurants and other food handlers.

The following inspection reports were completed Jan. 15-18.

A Loaf of Bread, 16 McGovern Ave., Jan. 16. Mixing bowl in mop sink. Corrected.

A Place to Feel Good, 1027 Dillerville Road, Jan. 16. No violations.

Arby's No. 7074, 888 E. Main St., Ephrata, Jan. 17. Cardboard Dumpster lid is damaged and is not tight-fitting. Replace to prevent vector entrance.

Arch Street Center, 223 W. Orange St., Jan. 18. No violations.

Babbo's Italian Grill, 2000 W. Main St., Mount Joy, Jan. 17. Drink cups without handles stored in bulk dry ingredient containers. Three large containers of soup made the previous evening were left to cool at room temperature overnight. Temperature varied between 75-85 F. Deeply scored cutting boards not resurfaced or discarded as required.

Barshinger Center Life Sciences and Philosophy Cafe, 600 College Ave., Jan. 17. No violations.

Black Knights Tavern & Grill, 335 Main St., Landisville, Jan. 16. Follow-up inspection; no violations.

Brubaker's Nuts and Dried Fruit, 955 N. State St., Ephrata, Jan. 18. No violations.

Cocalico High School, South Fourth Street, Denver, Jan. 16. No violations.

Family Dollar No. 0989, 130 N. Reading Road, Ephrata, Jan. 17. No violations.

Golden Mart, 2270 New Holland Pike, Jan. 15. The person in charge does not have adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. The person in charge did not demonstrate adequate knowledge of the state Food Code as evidenced by incorrect responses to food safety questions. Detergent stored on the same shelf with food equipment and single-service articles in the food-prep area. Food employees in the food-prep area not wearing proper hair restraints such as nets, hats or beard covers. Shell eggs stored over ready-to-eat foods in the reach-in cooler. Food stored directly on the floor in the walk-in cooler and in the customer service area rather than 6 inches off of the floor as required. Consumer self-service display of hot dogs without sneeze guards or other effective protection. Sausage and bacon sandwiches were held at 109 F in the hot-holding area rather than 135 F or above as required. Deli meats, tomatoes and cheese were held at 46 F in the bain marie rather than 41 F or below as required. Several cracked plastic inserts used for food storage. Old, tattered alumninum foil taped to shelves making them not easily cleanable. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Metal inserts are greasy and not clean to sight and touch. Pink slime noted on the drop plate of the ice maker. Old food residue on the ham slicer in the food-prep area. Heavy accumulation of old food splatter in the microwave in the food-prep area. Old, unused equipment stored throughout the facility should be removed.

Kitchen Collection No. 170, 632 Stanley K. Tanger Blvd., Jan. 16. No violations.

McDonald's No. 06636, 1434 Manheim Pike, Jan. 16. Working containers of cleaner/sanitizer were stored on the same shelf with food and equipment in the food-prep area and in the customer area at the self-serve soda machine. Wet wiping cloths in the food prep area and the customer area not being stored in sanitizer solution. Mechanical warewashing equipment with buildup of filth and food residue not cleaned before use and frequently throughout the day. Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low-temperature dishwasher was 0 ppm and not 50-100 ppm as required. Heavy accumulation of white slime on the ice maker drop plate. Old food splatter inside microwaves and old food residue on handles, doors and outer surfaces of nonfood contact areas. Clean food equipment in the dishwashing area stored wet in a manner that does not allow for draining and/or air drying. Clean dish racks stored on the floor in dishwashing area. Single-use items (to-go boxes) stored with food contact surface exposed. Food employee personal belongings -- coats, jackets, phones -- in the food-prep area, although lockers are provided.

Nanak Enterprises/Gas Mart, 653 Columbia Ave., Jan. 16. Creamer machine running high. Employee asked to monitor. Nonfood contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Ceiling tiles need cleaned or replaced by next inspection.

Plum Street Grocery (part 2), 599 N. Plum St., Jan. 18. No violations.

Starting Gate Tavern, 552 St. Joseph St., Jan. 15. Clean up trash in rear. Cover broken window at crawl space and clean up clutter in basement, which presents a fire hazard.

Target T-2072, 2385 Covered Bridge Drive, Jan. 16. Food facility has insufficient cooling equipment to cool foods within the six hours required. Hand-sink soap and paper-towel dispensers located over food-prep sink. Gallon jug in the hand sink, indicating uses other than handwashing. Corrected.

The Restaurant at Plain & Fancy Farm, 3121 Old Philadelphia Pike, Bird-in-Hand, Jan. 16. No violations.
