Ephrata Area School District announces Kindergarten registration

Ephrata Area School District will hold kindergarten registration sessions for the 2013-14 school year from 2 to 6 p.m. March 4 and 6, and from 3 to 7 p.m. March 12.

Children who will be 5 years old on or before Sept. 1 are eligible to be registered.

Parents should bring the following documents:

n Proof of residency. Examples include a utility bill, a lease, a housing contract or a sales agreement.

n Child's birth certificate.

n Biological parent's photo identification. Examples include a driver's license or a passport.

n Proof of immunizations. For a list of immunization requirements, go to www.easdpa.org/HealthServices/ Immunizations.html. Parents also can call the nurse at any of the district's elementary schools.

Parents should bring one of the following as proof of the immunization a child has received to date:

1) A copy of a child's immunization record provided by a healthcare provider.

2) A copy of a child's growth/baby book with recorded dates of all immunizations.

n Child's Individual Education Plan (if applicable).

n Physical and dental examination forms.

Eligible children will be required to have a physical and dental examination. The necessary forms will be provided at the time of registration. Exams must be completed and forms returned to the appropriate elementary school before Oct. 31.

Incoming kindergarten students who have completed the registration process on any of the aforementioned dates will be eligible to participate in a screening session in the spring. Parents/guardians will be notified in early May of their child's screening date and time.
