Food facility is reusing plastic mushroom containers, which are intended to be a single-service article. Pieces of silicon glued to a metal plate in walk-in cooler are loose. Correct so that it is a smooth surface.

Several raw animal foods were stored above ready-to-eat foods in the reach-in coolers. Common cups being used in bulk foods instead of using scoops. Women's restroom is not provided with a covered waste receptacle for sanitary napkins. Paper towel dispenser empty at the hand sink in the food prep area. Employee clothing being kept on top of food items on food racks. Mops are not being hung to air dry.
